DESCRIPTION:

Provide individualized drug therapy recommendations to optimize patient outcomes in assigned service/program at HDH/KGH.  Ensure that drugs are procured, stored, prepared, prescribed, dispensed and administered in a safe and effective manner.  Participate in education and research activities related to area of practice.

 

Within this role the employee is accountable for contributing to the delivery of the Kingston General Hospital strategy.  As an employee, one must demonstrate an awareness of and be responsible for actively promoting and supporting patient and family centered engagement and care in all we do.

 

 

PRINCIPAL RESPONSIBILITIES & DUTIES:

Patient Care

  • Function as a clinical pharmacist in assigned patient care service or program.
  • Identify, resolve and prevent drug-related problems using a patient-centered, collaborative approach.
  • Provide drug information and therapeutic recommendations to health care professionals and intervene on the patient’s behalf, as necessary.
  • Participate in interprofessional activities including informal and formal teaching rounds and discharge management.
  • Report adverse drug reactions according to hospital policy.
  • Perform patient medication histories, medication reconciliation and discharge medication counselling.
  • Conduct medication profile rounds on patient care service.
  • Interpret serum drug levels and make recommendations.
  • Document care provided in the patient’s chart.
  • Monitor drug use on assigned service and participate in drug use evaluations.
  • Participate on assigned departmental, program and hospital committees.

 

Drug Order Review/Entry and Dispensing

  • Review drug orders for completeness, accuracy and appropriateness.
  • Identify and communicate actual and potential drug related problems to service pharmacist or physician as appropriate.
  • Supervise pharmacy technicians responsible for entering drug orders in the Pharmacy information system and receiving, preparing, checking, and dispensing drugs, on a rotational basis including evening and weekends.
  • Enter parenteral nutrition and chemotherapy orders in the Pharmacy information system, prepare parenteral nutrition worksheets and assist pharmacy technician with drug order entry as required

 

Education

  • Provide patient and family counseling and teaching on proper use of medication, compliance and other related information.
  • Act as preceptor to pharmacy students and residents by directing, supervising, and evaluating activities and patient care provided.
  • Assist in orientation and ongoing education of pharmacists and technicians.
  • Provide interprofessional education on timely and relevant topics in drug therapy.
  • Participate in pharmacy clinical rounds and relevant medical in-services and continuing education activities.


Research

  • Review clinical drug trial protocols and procedures pertaining to assigned program or service and participate in dispensing of investigational drugs.
  • Participate in the development of protocols, guidelines, pre-printed orders/order sets and research projects improving patient care and safety in assigned program or service.
  • Contribute to the pharmacy and/or medical literature.
  • Contribute and review articles for the Drug Information and the Medication Safety Bulletins
  • Participate in pharmacy practice research projects as assigned.

 

 

QUALIFICATIONS:

  • Bachelor of Science in Pharmacy  or undergraduate pharmacy degree recognized by the Pharmacy Examining Board of Canada (PEBC)
  • Licensed as a Registered Pharmacist with the Ontario College of Pharmacists (OCP) in part A, within 12 months of starting employment at KGH
  • Accredited Canadian Pharmacy Residency Certificate preferred.
  • Minimum of two years experience within the last five years in hospital pharmacy practice preferred
  • Experience in direct patient care, drug information, pharmacokinetics and computerized drug order entry
  • Strong knowledge of drugs and therapeutics with ability to apply in the provision of patient care
  • Demonstrated proficiency as a hospital pharmacist based on CSHP and OCP Standards of Practice and in NAPRA Professional Competencies
  • Demonstrated ability to make sound professional decisions in a complex interprofessional environment.
  • Excellent interpersonal skills with demonstrated ability to communicate effectively, courteously, and tactfully.
  • Strong team skills with a proven ability to perform duties independently, with a high degree of motivation and minimal supervision
