PRIMARY FUNCTION:

Under the direction of the Program Manager or delegate, the Registered Practical Nurse (RPN) is accountable to the public and responsible for ensuring that practice and conduct meets legislative requirements and the standards of the profession, as defined by the College of Nurses of Ontario. RPNs’ practice will also comply with Kingston General Hospital’s policies and procedures. The RPN autonomously provides care within a full scope of practice as outlined in the College of Nursing Standards of Practice

 

Within this role the RPN is accountable for contributing to the delivery of the Kingston General Hospital strategy.  As an employee, one must demonstrate an awareness of and be responsible for actively promoting and supporting patient and family centered engagement and care.

 

 

PRINCIPLE RESPONSIBILITIES:

Patient/Family Care

The RPN, as a member of the interdisciplinary team, and using the best available evidence, will assess, plan, implement, and evaluate the nursing care delivered to patients and families within the RPN scope of practice including outcomes, specific interventions and modifying the plan of care, accordingly.

  • Prioritizes patient care based on individual patient needs
  • Recognizes deviations from predicted patient responses and consults appropriately
  • Reports all changes in patient conditions, monitoring the changes with accuracy, and continuing to consults appropriately with the RN and other health care team members
  • Develops plans of care and coordinates care to achieve identified patient goals
  • Meets immediate identified nursing care needs
  • Assigns, teaches and supervises unregulated care providers (UCP)Teaches and delivers elements of established health programs to meet the bio/psycho/social, cultural, emotional and health learning needs of the patient and/or family
  • Exhibits critical thinking within the RPN scope of practice

 

Team Relationships

The RPN is an integral part of the interdisciplinary team who collaborates with all team members about patient needs.

  • Contributes to the overall functioning of the care team/unit/program
  • Collaborates with team members to ensure continuity of patient care
  • Identifies and is involved in ethical discussion related to patient care
  • Interacts with other units/programs/teams/departments to ensure quality patient care and appropriate resource utilization
  • Participates in discharge planning for patients in collaboration with the patient/family and interdisciplinary team members

 

Quality Improvement

The RPN is involved in quality improvement initiatives, to meet the needs of our patients, families and groups.

  • Identifies opportunities for improvement related to safe patient care and service delivery
  • Involved in continuous quality improvement initiatives

 

Professional Development/Accountability

The RPN is responsible and accountable to identify and respond to personal professional development needs and opportunities.

  • Maintains membership in appropriate professional organizations
  • Maintains competence in nursing practice and enhances professional growth through lifelong learning
  • Identifies annual learning opportunities and goals as part of reflective practice
  • Adheres to legal and ethical principles relevant to nursing practice
  • Meets legislative requirements for annual updates i.e. WHMIS
  • Participates in and supports the development of a quality practice environment
  • Acts as a mentor, coach, clinical resource and consultant for RPNs, practical nurse students and UCP’s to promote excellence of care

  • Demonstrated knowledge of the CNO standards that apply to the RPN scope of practice in the area of hire.
  • Excellent organization and communications skills
  • Sound problem solving, conflict resolution and decision making skills
  • Ability to function independently within scope of practice
  • Basic computer skills
  • Ability to cope with stressful work environment
  • Proven ability to attend work regularly.
  • Satisfactory criminal reference check and vulnerable sector search required.
