Developmental Pathways has an opportunity for a Coordinator in the Community Engagement department. The Coordinator will plan, coordinate, support, and monitor locally funded and other program support services for adults and/or children living with disabilities in a variety of settings. The Coordinator will assist with department initiatives and outreach efforts.
 
 
The ideal Coordinator will communicate and interact in an appropriate, effective, and respectful manner with a broad team, which may include individuals in service, their family members and/or guardians, medical and health providers, professional supports, direct care employees, case managers, and natural support chosen by the individual/family being served.
 
 
This role offers a 4-day, 40-hour workweek, Monday-Thursday, and includes the flexibility to work remotely, as well as the opportunity to collaborate in-person at our headquarters office in Aurora, CO.

Requirements
  • An Associate’s degree or equivalent experience in a human services related field is required;
    • OR five (5) years’ experience in the field of developmental disabilities,
    • OR a combination of education and experience appropriate to the requirements of the position
  • Ability to meet and maintain our driving requirements; no suspensions and less than three (3) tickets in three years
  • Personal vehicle in good operating condition for use during work as appropriate
  • Valid Colorado motor vehicle license and proof of motor vehicle insurance
 
Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation. 
 
Preferred
  • Bachelor’s Degree
  • Two (2) years’ experience in service coordination/case management at Developmental Pathways or similar agency
  • Working knowledge of the DD system, rules, and regulations
  • Able to speak, write, and interpret fluently in English and another language based on the business needs
  • Ability to complete all typical case management responsibilities in fluent language/s
  • Support with written translation of internal documents for use by stakeholders, as appropriate


Additional Responsibilities
  • Research, request, and obtain potential services and /or providers to meet individual needs/ preferences as determined by identified documentation including needs assessments
  • Assure expenditures are within budgetary guideline
  • Mentor individual’s service choices, and help families determine priorities for service
  • Monitor health and safety issues, advise and counsel as necessary; facilitate referral to other agencies and disability programming and prepare the family for transitions to other services as appropriate
  • Assist with initiatives, meeting outreach goals, and with events such as Resource Fairs, Holiday Outreach Projects, other department or agency events, etc. Occasionally attends and/or presents at resource fairs, school liaison opportunities, and community events as assigned to communicate information about programs and services
  • Integrate knowledge of rules, regulations, and requirements as related to serving diverse populations and the coordination of unusually challenging and complex caseload issues; problem-solve program and other issues with the management team, providers, families, and other Coordinators
  • Serve as liaison to other Pathways departments and outside entities for a variety of reasons such as to interpret documents, request information and data updates, promote smooth transitions and convey program and policy changes
  • Provide coverage as needed for ad hoc customer service issues; helps coordinate resolutions for difficult situations
  • Provide support for completed Unmet Need requests including obtaining supporting documentation and completing purchasing documentation
  • Ensure paperwork submitted by service providers, other individuals, and other agencies is completed, distributed, and filed
  • Enter data in required tracking tools and databases accurately and punctually; generate database reports
