Unit Manager

Current NYS RN license; 2-3 years nursing in long-term care; prior supervisory experience preferred; strong leadership and communication.

MDS Nurse

RN license; 1–2 years MDS experience in long-term care; CMS regulations knowledge; proficient EMR use.

Cook

Healthcare or long-term care cooking experience; knowledge of therapeutic diets; read recipes; food safety; teamwork.

CNA - Quality Staffing

Certified Nursing Assistant

Current CNA certification; experience in long-term care preferred.

Licensed Practical Nurse (LPN) with clinical nursing skills and ability to supervise nursing staff.

Fiscal Clerk 

High school diploma required; associate degree in accounting, business, or related field preferred; experience in clerical/fiscal/administrative roles; proficient with Microsoft Office; strong accuracy and confidentiality.

Physical Therapist 

Licensed Physical Therapist with state licensure, CPR, and experience in long-term care; strong assessment and documentation skills; PDPM knowledge preferred.

Physical Therapy Assistant

Licensed Physical Therapist Assistant with NY license; two-year PTA program graduate; minimum six months experience preferred.

Provide direct patient care as a Certified Nursing Assistant, assisting with ADLs, vital signs, safety, and documentation.

Licensed Practical Nurse

Licensed Practical Nurse needed to supervise CNAs, provide direct resident care, perform medication administration, and assist with care planning and documentation.

Nursing Home Administrator

Must have Nursing Home Administrator license in NY, 3+ years LTAC experience, strong budget, staffing, and regulatory compliance skills.
