Supervises staff, ensures compliance, MS Office skills, high school diploma, 1 year related experience, valid driver license, ability to lift 45 lbs, flexible schedule.

INTERRAI ASSESSOR

InterRAI Assessor

Bachelor’s degree in Human Services; 18 months experience completing assessments and with Developmental Disabilities; valid driver license; working with Medicaid and county programs; proficient computer and electronic documentation skills.

Licensed Ohio RN or LPN; 1+ year experience; valid driver license with <6 points; ability to lift up to 45 lbs; on-call rotation; weekend work.

Licensed practical nurse with current Ohio LPN license; associate degree preferred; 1+ year experience; valid driver license; ability to lift up to 45 lbs; nurse-on-call duties; works weekends.

Direct Support Professional

High school diploma; valid driver’s license; ability to perform light to moderate physical labor; lift up to 45 lbs; some holidays required.

LICENSED PRACTICAL NURSE INTERMITTENT-ON-CALL

Licensed Practical Nurse

Current Ohio LPN license, 1+ year experience with individuals with developmental disabilities; valid driver’s license; ability to lift up to 45 lbs; on-call rotation; weekend availability.
