Care Management Specialist - All Shifts

Care Management Specialist

At least 1 year of experience in human services; HS diploma; ability to pass background check and drug screen; knowledge of SUD treatment and social services; eligible for Medicaid billing; strong communication.

Licensed SUD/MH Counselor - PHP Program

Licensed SUD/MH Counselor

Master's degree in human services or related field; two+ years counseling/social work experience; LCDC2 or higher; urine drug screen required.

Maintenance Technician

Facilities maintenance experience; 2+ years; high school diploma; valid driver’s license; able to pass background checks and drug screen; HVAC/plumbing/electrical/carpentry skills; able to manage work orders and supervise crews.

Care Management Specialist - Weekends 

High school diploma or equivalent; 1+ year experience in human services; background check and drug screen; knowledge of SUD, healthcare, social services, and criminal justice; strong communication and crisis handling skills.
