Patient Navigator

Women's Health APP (PA/NP/CNM) - Full Time 

Women’s Health APP

Advanced practice provider for outpatient obstetric and gynecologic care; requires WHNP or CNM training; CT licensure eligibility; strong communication and team skills.

Street Medicine Physician

Per diem physician providing medical/surgical care with harm reduction approach; must be board eligible/certified; Connecticut license; admitting privileges at local hospitals; CPR certified.

Referral Specialist

High School Diploma/GED required; 6+ months related experience; customer service experience; medical terminology knowledge preferred; strong communication and computer skills; bilingual Spanish preferred; valid CT driver’s license and daily car access.

Registered Dietitian

Dietitian

Bachelor's degree in nutrition/dietetics; RD/RDN credential; active status; CDE if applicable. Experience in outpatient nutrition care and diabetes education preferred.

Advanced Practice Provider

APRN or PA with board certification or eligibility; CT licensure; at least 5 years outpatient internal medicine/family practice; CPR; addiction medicine experience preferred.

Director of Finance

MBA/MPH/MHA or master’s in health care-related field preferred; bachelor’s in Accounting or Business Management required; CPA highly desirable; minimum 3 years as CPA or senior accountant; experience in large healthcare finance.

Clinical Pharmacist - Per Diem

Clinical Pharmacist

Per Diem Clinical Pharmacist; CT licensed; degree from accredited college of Pharmacy; residency preferred; ASHP/organization and Joint Commission experience preferred; strong communication with staff and patients.

LPN with CT licensure; experience in Internal Medicine or related fields; CPR; phlebotomy and EKG experience; bilingual Spanish a plus; Centricity/Yale EPIC skills.
