Intake Case Manager, The Cradles Project

Intake Case Manager

Bachelor’s in social work or related field; LCDC-I or LCDC preferred; bilingual preferred; 2+ years behavioral health/case management; experience with vulnerable families; strong resource coordination; reliable transportation.
