Care Coordinator (Case Management)

Care Coordinator

Provide targeted mental health case management for youth; assess and plan care; coordinate with families and providers; ensure documentation and crisis response.

Bachelor's in mental health field with 1 year mental health experience, or bachelor's in another field with 2 years mental health experience; valid driver's license; flexible schedule; training/certification per CSA and BHA; Wraparound certification.

Licensed Mental Health Therapist - Part Time & Full Time

Mental Health Therapist

Active master’s level license in Maryland (LMSW/LGPC/LCSW/LCSW-C/LCPC); at least 1 year of clinical experience; strong communication; commitment to diverse populations.
