QAPI Registered Nurse Manager

RN license required; BSN required; MS preferred; 3-5 years in Home Health/Hospice QA or leadership; supervisory experience.

Home Health RN

Part-time Home Health Registered Nurse with BSN, IL RN license, CPR, driver’s license; home health experience preferred; strong clinical and customer service skills; 3 days/week (Mon/Wed/Fri).

Registered Nurse

BSN, Illinois RN license, CPR, valid IL driver’s license, home health experience preferred, excellent nursing and customer service skills.

Physical Therapist- Home Health

Physical Therapist

Licensed Illinois Physical Therapist with CPR certification; drivers license with auto insurance; degree in physical therapy (Bachelors or Masters). Home Care Home Base experience preferred.

Family Home Health and Hospice

Hospice Biller

2 years medical billing experience; Medicare/Medicaid and commercial billing; HIPAA/compliance; strong verbal/written communication; detail-oriented; MS Office; Home Care Home Base experience a plus.

Physical Therapist- PRN

Licensed Physical Therapist in Illinois; CPR; driver’s license and auto insurance; per diem scheduling; Home Care Home Base experience preferred.

Bachelors or Masters in physical therapy; Illinois PT license; CPR; IL driver’s license and auto insurance; Home Care Home Base experience preferred.

Part-time physical therapist with Illinois license; Bachelors or Masters in physical therapy; CPR; valid IL driver’s license; EMR experience preferred.

Speech-language pathology degree; IL license; CPR; valid IL driver's license; Home Care Home Base experience preferred; 1 year experience.

Home Health RN or LPN- PRN

Home Health Nurse

Must possess current Illinois RN or LPN licensure; CPR; driver license; home health experience preferred; HCHB EMR experience a plus; strong clinical nursing and customer service skills.
