Caregivers (HHA) & PCA'S - 4 or 6 Hour Shifts

Caregiver

Provide direct patient care under supervision; assist with ADLs; mobility/transfers; monitor vitals; maintain safe environment; communicate with patients and families.

Registered Nurse, Home Health, OASIS Experience Required

Registered Nurse

Two years of home health experience; CT RN license; reliable transportation to commute to patient homes.

Caregivers (12 Hour Days) 5+ Years Experience

Caregiver with 5+ years experience; assist with daily living; monitor vital signs; valid driver’s license.

Physical Therapist (Per Diem)

Physical Therapist

Licensed PT in Connecticut with 3+ years of experience; strong communication; able to manage a flexible caseload.

Home Health Registered Nurse- Every other Weekend

Active CT RN license, minimum 2 years nursing (home health preferred), reliable transportation, strong communication and organizational skills, knowledge of OASIS, BLS preferred.

On-Call Hospice Registered Nurse

Graduate of accredited nursing program; current RN license; minimum 2 years RN experience; able to work independently and supervise others; hospice training; strong communication and crisis response skills.

Hospital Liaison

Coordinate patient care by liaising among physicians, hospitals, patients, and community resources; screen referrals; ensure continuity of care; requires CT license and background check.

Physical Therapist Assistant

Assist licensed physical therapists; provide patient care; monitor progress; document notes; work in home health or skilled facilities.

Hospice Registered Nurse

Licensed RN in CT with hospice experience; graduate of an accredited nursing program; minimum 2 years RN experience preferred.

Caregivers - CNA, HHA, PCA - Mornings/Afternoons- Week/Weekends- Stamford & Greenwich

Provide direct personal care in home setting; CNA/HHA/PCA duties; must have HS diploma or GED; CNA certification; CPR; valid driver's license; good communication and compassionate demeanor.

Valid Physical Therapist license; home health experience preferred; knowledge of anatomy, physiology, and medical terminology; strong communication; patient-centered care.
