RN Inpatient - Nights

RN Inpatient

Registered nurse with Ohio license; BSN preferred; 1-2 years clinical experience; acute setting preferred; EMR experience (Netsmart) preferred; CPR certified; strong communication and teamwork.

Annual Giving Director

Bachelor's degree; 3-5 years direct fundraising (annual giving/donor relations); strong MS Office; Blackbaud Raiser's Edge preferred; strong organizational, analytical, planning, and communication skills.

LPN Homecare

Weekend-only LPN position; 24 hours/week; Ohio LPN license with ability to obtain Michigan license; EMR experience; CPR; strong communication and organizational skills.

STNA - Inpatient Nights

Must be a state-tested nursing assistant (STNA); part-time position, 24 hours per week; inpatient nights; ability to provide patient care.

Dietary Service Associate

High school diploma; 1 year cooking/food service experience; ServSafe Level 1 or within 90 days; strong communication; ability to read recipes; teamwork; able to work shifts in Toledo and Perrysburg.

STNA - Inpatient

Must be a State Tested Nursing Assistant (STNA); test-ready option may be available with employer-paid testing within 90 days.

RN Case Manager

Registered nurse with current Ohio license; BSN preferred; 2+ years in medical-surgical nursing; home/hospice experience preferred; EMR experience; strong communication; valid driver with reliable vehicle; CPR certified.

Gardener

High school diploma; 2 years experience preferred; ability to lift 25 lbs; valid driver with insured vehicle; good communication; independent work; hospice philosophy understanding.

Active RN license in Ohio; ability to obtain Michigan license; BSN preferred; 2+ years medical-surgical nursing; hospice/homecare experience preferred; EMR experience (Netsmart); CPR; valid driver with reliable vehicle; strong communication; independent and team work.

Must be a State Tested Nursing Assistant (STNA), or test-ready; employer pays the cost of the test if obtained within first 90 days of employment.

Physician Palliative Care

MD/DO with current Ohio and Michigan license; board certified in family medicine, internal medicine, oncology, emergency medicine, or related specialty.

Advanced Practice Nurse - Palliative Care

Advanced Practice Nurse

APN program graduate; unrestricted Ohio and Michigan APN and RN licenses; adult/family/gerontological specialization with CE; 3-4 years APN experience; EMR experience (NetSmart preferred); valid driver's license; CPR; strong communication; able to handle emergencies.

Licensed Massage Therapist - Independent Contractor

Licensed Massage Therapist

Provide massage therapy to hospice patients; participate in care teams; document and report care; provide staff training.

RN licensed (OH license required) with bonus if MI license; BSN preferred; 2+ years medical-surgical; homecare/hospice experience preferred; EMR Netsmart experience; CPR certification.

Hospice Physician - Conting

Hospice Physician

MD/DO with license in OH and MI; hospice/palliative board certification; completed UNIPAC or will within 6 months; hospital medical staff membership; 3+ years medical experience; current DEA; valid driver; EMR experience (Netsmart/EPIC preferred).

APN with OH/MI license, 3-4 years hospice/home health experience, EMR (NetSmart) proficient, CPR certified, driver with reliable vehicle.

RN Homecare On-call

RN with Ohio license; Michigan license allowed; BSN preferred; 2 years med-surg; homecare/hospice preferred; Netsmart EMR experience.
