Director of Nursing (DON)

Director of Nursing

RN license in Tennessee; 1 year DON or 2 years ADON; knowledge of state/federal regulations; experience in skilled nursing facility; staff education; ability to work in fast-paced environment; focus on reducing hospitalizations; attention to detail.

Social Services Assistant

3+ years in social work; BSW/MSW preferred; flexible schedule; PCC experience preferred

Physical Therapist PT

Physical Therapist

Licensed Registered Dietitian with ability to work independently and mentor staff in geriatric nutrition.

CNA certification required; assists with daily living activities; shifts 7A-7P and 7P-7A.

MDS Coordinator

Licensed nurse with active license; minimum 2 years clinical healthcare experience; at least 1 year in long-term care; experience coordinating MDS.

Speech Language Pathologist SLP

Speech Language Pathologist

Licensed Speech-Language Pathologist in TN; geriatric/long-term care experience preferred; able to evaluate and treat independently; strong documentation.

Weekend Respiratory RN

Respiratory Therapist / Respiratory Nurse

Licensed RT or RN with rehab center experience; strong communication; weekend work; post-acute/long-term care setting.

Dietary Cook

1 year cooking experience; healthcare facility preferred; high school diploma or GED preferred.

Weekend Licensed Respiratory Therapy

Respiratory Therapist

Part-time respiratory therapist with rehab center experience; able to assess, diagnose, develop and implement respiratory therapy plans; strong communication.

Physical Therapy Assistant PTA

Physical Therapy Assistant

Licensed Physical Therapy Assistant; able to evaluate and treat independently; licensed in state of practice.

Certified Occupational Therapy Assistant COTA

Certified Occupational Therapy Assistant

Valid state license and registration; experience with long-term/geriatric care preferred; strong documentation; collaborates with disciplines; punctual and professional.

Housekeeper/ PT.
