Care Coordinator

Bachelor’s degree in Social Work, Nursing, Psychology, or related human services field; 2+ years with serious mental illness or chronic conditions; fieldwork experience; strong communication; trauma-informed care.

ACT Team Vocational Specialist

Bachelor’s degree preferred in Rehabilitation Counseling, Social Work, Psychology or related field; 1–2 years in vocational services or behavioral health; valid driver’s license; IPS knowledge preferred.

Health Home Unit Supervisor (Brooklyn-Queens)

Health Home Unit Supervisor

Bachelor’s degree in Social Work, Psychology, Sociology, Education, Nursing, Human Services, Healthcare Management or related field; 3+ years case management with 1+ year supervisory experience; SMI population and shelter experience; proficient in Microsoft Office; valid driver’s license a plus.

ACT Team Leader

Minimum 2 years supervisory experience in behavioral health; preferred ACT/mobile crisis oversight; clinical experience with SMI/SUD; coordinate housing, medical, educational and employment needs.

Community Resource Coordinator (Part-Time)

Community Resource Coordinator

Part-time role requiring HS diploma, bilingual English/Spanish, valid driver’s license, computer skills, able to lift 50 lbs, and travel across counties.

Care Coordinator (Brooklyn)

Bachelor’s degree required; one year of related human services experience; travel required; strong communication and organization; bilingual preferred.

Bachelor’s degree required; 1 year human services experience; travel; bilingual preferred; strong communication and advocacy.

Wellness Liaison

Experience in outreach and intakes; field-based; requires a car and a clean driver's license; bilingual Spanish preferred.

Intake Specialist

High school diploma required; strong written communication; good organizational skills; ability to work independently and in a team; fieldwork travel between Brooklyn and Queens; bilingual a plus.
