Southwest LTC

Certified Medication Aide with current CMA certification; high school diploma or equivalent; prior LTC/Skilled Nursing/assisted living experience preferred; knowledge of medication administration, safety, and documentation; strong attention to detail, compassion, and communication.

Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN)

LPN/LVN licensed in state of employment; graduate of accredited practical nursing program; long-term care experience preferred; CPR certification required.

Resident Transport

Organize, plan and supervise care; strong interpersonal skills; provide emergency care; maintain neat appearance; work with geriatric residents; follow care plans.

Housekeeper / Laundry 

Housekeeper

High school diploma preferred; healthcare/long-term care hospitality housekeeping experience preferred; knowledge of cleaning chemicals and infection control; ability to follow safety protocols and work independently or in a team.

Maintenance

High school or equivalent education with 2-5 years of building maintenance experience or related skilled trades training.

Dietary Aide

High school diploma preferred but not required; experience in dietary services or healthcare preferred; knowledge of safe food handling; ability to lift up to 25–50 lbs; team player.

Current CMA certification, high school diploma, experience in long-term care preferred, knowledge of medication administration, safety, and documentation.

Maintenance Assistant

General building maintenance; carpentry, plumbing, electrical work, painting; HVAC and safety compliance; team collaboration.

Knowledge of building maintenance, codes, safety; HVAC, plumbing, electrical; painting, carpentry, flooring; operate and repair mechanical systems.

Cook - Part Time

Cook

CNA Double Weekend

Certified Medication Aide responsible for medication administration, documentation, and resident care per care plans.

Leads and supervises nursing staff; administers meds and treatments; ensures resident care plans are followed; strong communication and organizational skills.

Provides nursing leadership, supervises CNAs, administers meds, coordinates care, ensures HIPAA compliant documentation.

Provide resident care per interdisciplinary care plans; assist with daily living; maintain hygiene and dignity; document care; communicate with licensed staff.
