Case Manager

RN or LPN license; Bach or Associate degree with Human Services experience; 2+ years in Human Services; 50-75% travel; background checks.

Attendant Care Aide

Experience in hospice, home health, or related fields; valid driver’s license; pass drug screening and background check; positive demeanor; ability to build rapport; able to handle physical workload.

Program Aide

Full-time Program Aide with compassion for the elderly; CNA preferred; experience in adult day health care; background check and drug screening required.

Provide routine in-home care to elderly or disabled; requires hospice/home health experience, valid driver’s license, ability to pass drug/background checks, and good rapport.

Provide non-medical in-home care for elderly or disabled, including personal care, light housekeeping, meals, errands; must pass drug screening and background check.

Coordinate care for elderly/disabled clients; assess needs; develop care plans; requires relevant human services experience.

Bachelor's degree in Social Work, Human Services, or related field, or related experience; various credentials may apply; about 50% local travel.

Bachelor's in Social Work or Human Services, or related field; or Bachelor's with 1+ year HS experience; or Associate in Behavioral/Social Science with 2 years HS experience; or Kentucky-licensed RN with 2+ years aging/disabilities; or Master’s in Health or Human Services.
