Description
We are seeking a compassionate and highly skilled Nurse Practitioner specializing in Hospice and Home Health to join our interdisciplinary care team on a per diem basis . This role provides advanced clinical care to patients across the continuum from chronic illness management in the home health setting to comfort-focused, end-of-life care in hospice.
The Nurse Practitioner works under the supervision of a collaborating physician and in accordance with the state Nurse Practice Act, delivering patient-centered care in homes and community settings. This role emphasizes clinical excellence, care coordination, and compassionate support for patients and families.
Compensation
- $100 per visit for all completed patient visits (home health and hospice encounters)
- $30.00 per hour for all Interdisciplinary Team (IDT) meetings and orientation/training sessions
Key Responsibilities
Clinical Care – Hospice & Home Health
- Provide comprehensive medical care, including assessment, diagnosis, treatment, and symptom management for patients in home and hospice settings
- Perform detailed history and physical examinations and document findings in the medical record
- Order and interpret laboratory tests, imaging, and other diagnostics
- Develop, implement, and adjust individualized care plans based on patient needs and goals of care
- Prescribe medications, treatments, and therapeutic interventions within scope of practice
- Manage chronic, acute, and terminal conditions across home health and hospice populations
- Provide palliative care and symptom management for hospice patients under physician supervision
- Conduct hospice face-to-face eligibility and recertification assessments
- Recommend hospice admission, certification, and recertification to the medical director
Care Coordination & Communication
- Collaborate with physicians, nurses, social workers, therapists, and interdisciplinary team members
- Participate in regular interdisciplinary group meetings to review patient progress and care plans
- Communicate effectively with attending physicians, hospice medical directors, and community providers
- Ensure continuity of care between home health and hospice transitions
- Educate patients, families, and caregivers on disease management, treatment plans, and end-of-life care
- Provide clinical consultation and support to interdisciplinary team members
Documentation & Compliance
- Maintain accurate, timely, and complete clinical documentation in the medical record
- Ensure compliance with Medicare/Medicaid regulations, state/federal hospice and home health requirements, and organizational policies
- Follow HIPAA guidelines and maintain strict patient confidentiality
- Participate in quality improvement initiatives and clinical performance monitoring
Requirements
Qualifications
- Graduate of an accredited School of Nursing
- Completion of an accredited Nurse Practitioner program
- Current RN and Nurse Practitioner licensure in the State of practice (California preferred if applicable)
- Master’s degree required
- Minimum 1 year of hospice or palliative care experience required
- Home health experience preferred
- Current CPR certification
- Certification in hospice/palliative care (e.g., CHPN) or related specialty preferred
- Strong clinical assessment, communication, and critical thinking skills
- Ability to independently manage patient care in home and community environments
