Job Summary and Qualifications

The case management assistant (CMA) will work under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services and will identify and report barriers to discharges. The CMA will provide administrative support in Case Management functions. 

Major Responsibilities:

  • Coordinates with case manager and social worker for support needed
  • Provides clerical support in multidisciplinary rounds
  • Makes referrals through the web based referral system to the specific post-acute care (PAC) providers requested
  • Monitors patients with PAC referrals and intervenes to expedite discharge
  • Prioritizes workload to ensure a timely contact and intervention PAC providers
  • Delivers the Beneficiary Notice to patients identified as being in the BPCI-A program
  • Obtains physician signatures on required Case Management documents and forms
  • Based on family practice; sets up transportation/send information to Transfer Center and generates discharge packets for PAC transfers
  • Practices and adheres to the "Code of Conduct" and "Mission and Value Statement". 

What qualifications you will need:

  • High School Graduate / GED
  • Associate degree preferred
  • Medical knowledge and terminology preferred
  • CNA, EMT preferred
  • Minimum of 1 year experience in a healthcare setting

  • CNA or Medical Assistant experience preferred
  • Knowledge of medical terminology preferred
