OVERVIEW:

The case manager is assigned to directly oversee residents’/clients’ services, which includes, but not limited to; developing dynamic treatment goals in accordance with his/her individual program plan or equivalent. The case manager shall interact with clients on a daily basis, and utilize cognitive behavioral techniques to change maladaptive behaviors in order to foster and promote prosocial decision-making & healthy interpersonal skills. The case manager will be required to facilitate client group sessions on prosocial decision-making in structured and manual-driven classroom setting. The case manager is required to meet all regulatory compliance standards, inclusive of entering information into the agency case management system.

CASE MANAGEMENT:

Develop, initiate and implement treatment plans within the structure of the program.
Provide orientation to new clients.
Provide bi-monthly evaluation sessions at minimum.
To enlist and coordinate resources to meet individual client objectives, both internal and external to the agency.
To assist and direct inter-agency staff in meeting client needs.
To recommend and participate in contract evaluations and appropriate committees.

CASE REPORTING:

Records of all cases must be kept in an orderly, professional and confidential manner.
Records are to be maintained as per agency policy and include bi-monthly case entries for evaluations, phase movements, contracts, reports to referral agencies and significant case developments.
All cases terminated should have a summary sent to the referral agency within 72 hours of discharge.
Monthly reports are to be submitted to the Program Director summarizing cumulative counseling records, community resource utilization and other activities as required.

PROGRAM COORDINATION:

To meet with other program staff weekly to coordinate treatment activities.
To report to the Program Director of both client progress and staff effectiveness.
To assure internal management procedures are followed as it relates to agency polices.
To maintain contact with the agency of referral providing support services.
To contribute to the planning and implementation of activities within the assigned unit.
To perform other activities as assigned by the Program Director.
Attend all probationary training, unit meetings, and any additional training assigned.

QUALIFICATIONS:

Master’s degree, no experience, or Bachelor’s Degree with two years’ experience in the field of Social Services.
Knowledge of Criminal Justices system, behavioral process and integrated staff format expected.
Ability to relate with client population and coordinate treatment activities.
Must possess strong organizational skills and be detail-oriented.
Must possess excellent verbal and written communication skills.
Must possess a valid driver’s license and a driving record that doesn’t preclude the employee from being covered by agency liability insurance.
Maintain a record which does not preclude the employee from working with offenders as required by regulatory agencies.
Proficient with Microsoft Office or related software.
Must have 40 hours of training per year that pertains to job description.

PHYSICAL REQUIREMENTS:

Prolonged periods of sitting.
Frequently required to stand, walk, reach with hands and arms, and stoop, kneel, crouch or crawl.
Must be able to lift and/or move up to 25 pounds.
