TITLE: Dax Case Manager

HOURS: Part-Time, 24 hrs. per week.

COMPENSATION: $26.00 per hour, paid bi-weekly

Position Summary
The Dax Program provides housing and support to approximately 15 college students experiencing homelessness and housing insecurity in Denver. Most students are aged 18-25 and are referred directly from local universities. We offer housing, basic needs, and financial assistance within a trauma-informed, holistic, and participant-oriented framework. To meet the demanding schedules of college students, the Case Manager’s hours include occasional evenings and weekends.
Primary Duties and Responsibilities
Essential Job Functions
• Provides guidance and support for Dax residents. Develops support plans to coordinate services for residents on a regular basis (at least once per month) recording the work appropriately, advocating for students, recording notes in the database and updating the supervisor and the project team on their progress.
• Assesses the needs of residents and refers them on to appropriate agencies and social service programs both at their college and in the community.
• Completes referral interviews and screening with referred students; conducts intake assessment and develops housing move in and discharge plans.
• Serves as a liaison with all private and public social services agencies with particular regard to housing, food resources, health and benefits issues.
• Oversees social and community programming for Dax participants including events, workshops, seminars, completing grant applications, and tracking participation and feedback.
• Conduct outreach with local businesses, community-based organizations (CBOs), non-profits and community agencies to develop the Wellness program and volunteer opportunities for the Dax Program.
• Works with the program Director to manage social media for the Dax Program.
Additional Tasks and Responsibilities
• Prepare reports, statistics and other papers which may be required by the City Director and senior management team.
• Conducts data entry and maintenance on a computerized monitoring system.
• Conducts tours of the facility for visitors and volunteers.
• Ensure all appropriate records and forms are completed by residents.
• Maintains all records of work necessary for the smooth running of the project.
• Build good working relationships with other projects and agencies.
General Responsibilities
• Stays informed of current local and national events, trends, and issues that impact Depaul USA’s work with people experiencing homelessness.
• Strives continuously to develop the role in collaboration with the supervisor to ensure that all tasks are undertaken effectively and appropriately, meeting the strategic goals and objectives of Depaul USA.
• Participates in internal and external meetings as required, attends training events, conferences, and other functions as necessary.
• Participates in regular supervision and annual evaluations, identifying personal developmental and training needs.
• Ensures adherence to all Depaul USA policies and procedures, particularly those relating to Health and Safety, Safeguarding, and the Code of Practice and Confidentiality.
• Contributes to the effective implementation of Depaul USA’s Equal Opportunities Policy as it affects both the organization and its work with this participant group.
• Undertakes the role in a professional manner, maintaining a high-quality standard of work in accordance with the Vision, Mission, and Values of Depaul USA.
• Undertakes any other duties that may be required which are commensurate with the role.
Qualifications
The ideal Case Manager will be a professional with at least two years of experience in case management with vulnerable segments of the population. S/he will have experience working with individuals who have faced homelessness and struggled with issues of mental illness, cognitive disabilities, and addictions, as well people who are newly housed or employed. The Case Manager will also understand social work theories and methodologies, be familiar with relevant partner agencies, and comprehend the needs of individuals seeking services. The Case Manager should possess the following:
• Associate degree in social work, counseling, psychology or closely related field with at least two years’ experience working with populations affected by behavioral health diagnoses and homelessness.
• Exceptional communication, multi‐tasking, conflict management, and interpersonal
• Strong computer skills with all Microsoft Office programs.
• Ability to organize tasks, events, and people and delegate effectively.
• Ability to exercise independent judgment when appropriate and strong decision-making skills.

• Weekdays: 2 PM - 8 PM or Weekends: 10 AM - 3 PM (Minimum of 3 days per week).
