Position Summary/Responsibilities
The goal of the Permanent Supportive Housing team is to assist our clients in securing and maintaining stable, affordable rental housing in the community. The program adheres to a Housing First model, an approach that prioritizes providing and funding permanent housing in order to end the cycle of homelessness and allow individuals to pursue personal goals, actively participate in mental health treatment, and improve their quality of life. The Case Manager engages homeless individuals with serious mental illness and assists them to secure housing, navigate the disability application process, and apply for all financial entitlements. The Case Manager helps clients access treatment and rehabilitative services for integrated health and behavioral health care, thereby promoting wellness, reducing the frequency of unnecessary emergency department visits and hospitalizations, and reducing overall health care costs. A successful candidate will be compassionate, proficient in completing clinical documentation, knowledgeable about community resources, have good interpersonal skills, and enjoy functioning as part of a small, highly interactive team.
This position is funded by the on-going Permanent Supportive Housing Funding from the Department of Behavioral Health and Developmental Services (DBHDS).
Hiring Range
The anticipated base salary for this position is $43,022.07 to $45,890.21. Actual compensation offered within this range will be determined based on the candidate's experience, education, qualifications (knowledge, skills, and abilities), geographic location, complexity of role, and other relevant business considerations. We kindly ask that candidates do not discuss compensation until after an offer of employment is extended.
Requirements
- Education: Minimum of a high school diploma. An Associate's or Bachelor's degree in a human services field preferred.
- Experience: One year clinical experience working in the human services field preferred. Experience working with individuals diagnosed with a Serious Mental Illness (SMI) or the homeless population preferred.
- Licensure/Certifications: Qualified Mental Health Professional (QMHP) preferred.
- Background Check: Fingerprints, State and FBI criminal record reports, drug test, and central registry report will be required upon selection.
- Safe Driving Record: This position requires transporting clients or driving agency vehicles. Candidates must have a good driving record, which is defined as no more than two moving violations in the last three years and no criminal moving violations in the last five years.
Special Work Conditions
While performing the duties of this job, the employee will often sit, stand, walk, bend, stoop, and reach above their head. Use of hands for placing and pulling objects. The employee occasionally lifts and or carries up to 25 pounds and pushes or pulls up to 150 (wheelchair) pounds. Specific vision abilities required by this job include close vision (computer and research). Specific hearing required by this job includes normal tones. Travel up to 5%. Travel is local. Travels to various sites in the community for the purpose of monitoring supports in a variety of settings and participating in meetings with peers and community agencies.
