CDU was founded to address inadequate medical care in the region and to provide equitable medical education opportunities for underrepresented students.

CDU is strongly invested in the local and regional community. The university leads multiple partnerships and programs to provide equitable healthcare resources in a variety of South Los Angeles neighborhoods.

 

Summary

 

The Clinical Manager will be responsible for the ongoing management of the daily operations for the Black Maternal Health Center of Excellence (BMHCE) Community Health Worker (CHW) and Patient Navigator (PN) Programs. The Clinical Manager will work with the BMHCE Clinical Leads to establish essential protocols and workflows for the CHW and PN programs and participate in evidence-based and data-driven quality improvement efforts.  The Clinical Manager will provide administrative oversight of CHW and PN team members and coordinate with the Workforce Development Core for onboarding, training, workforce development, and workforce satisfaction efforts for this team.  The Clinical Manager will collaborate with the Data Core to ensure the integrity of patient and program data accessed by the CHW and PN teams. The Clinical Manager will also manage a caseload of BMHCE clients, providing patient navigation support and coordinating the development and management of client care plans.

 

Essential Duties and Responsibilities:

 

  • Manage the administrative aspects related to supporting, training, orienting, and evaluating the CHW PN team and wraparound services for BMHCE clients through the Activate the Village Model. 
  • Maintain communication with BMHCE partners and the BMHCE team to coordinate wraparound and educational services out of the Maternity home; and support home-related activities. 
  • Responsible for the management of regularly scheduled CHW and PN administrative interdisciplinary team meetings and reviews. 
  • Participate in weekly supervision and consult weekly with the Director of the Black Maternal Health Center of Excellence. 
  • Manage the quality of clinical communication within and across BMHCE programs/projects. 
  • Ensure compliance with clinical and case management policies and procedures. 
  • Maintain and follow through on the Quality Assurance/Quality Improvement (QA/QI) processes for clinical and case management functions and services. 
  • Responsible for timely review of Electronic Health Records System (EHRS)/Electronic Medical Record (EMR) notes and documentation. 
  • Serve on program and agency committees and task forces at the discretion of the Director of the Black Maternal Health Center of Excellence.   
  • Oversight of the intake process and assignment of BMHCE client cases as needed. 
  • Attend trainings as needed. 
  • Represent the Center at public and assigned forums. 
  • Understand and follow all CDU’s safety policies, procedures, and rules. 
  • Conduct intake assessments and directly interface, coordinate, and follow up with BMHCE clients. 
  • Aid in the coordination and facilitation of the Postpartum Support Group.  
  • Maintain confidentiality of all pertinent client information. 
  • Perform other duties as may become necessary or as directed. 
  • Assist in writing scientific papers for publication. 
  • Some weekend event attendance required.

EDUCATION:

LICENSES/CERTIFICATIONS (IF APPLICABLE):

  • Bachelor’s degree from a regionally accredited institution in a relevant field such as Public Health, Health Administration, Nursing, Social Work, Psychology, or related field. , or equivalent experience is required. Master’s degree preferred.

 

 

EXPERIENCE/SKILLS:

  • Three (3) to five (5) years of progressively responsible experience in the related area.
  • 2 years or more of Supervisory or leadership experience (program management, staff supervision, etc..)
  • Strong understanding of program management methodologies and tools.
  • Excellent leadership, communication, and interpersonal skills.
  • Experience working with pregnant and/or parenting individuals of color, or experience working in programs that serve that population.  
  • Demonstrated commitment to understanding and eradicating structural racial inequity; experience working in and with issues of racial equity, diversity, and inclusion. 
  • Experience working as a liaison with diverse stakeholders; ability to establish relationships easily; highly effective listener; and ability to work collaboratively with internal and external partners. 

 

SKILLS:

  • Excellent written and verbal communication skills. 
  • Program development, implementation, and evaluation.
  • Knowledge of community health and population health principles
  • Experience with data tracking and quality improvement.
  • Collaboration with community partners and healthcare professionals/providers
  • Grant management or funding program oversight.

 

Compensation:

  • $90,000 annual

  • Full-Time, Exempt
