JOB SUMMARY

The Community Health Representative, under the direction of the HHS Director, will provide community outreach services, health education, monthly workshops, home visits, health screenings, follow-up referral services, health promotion/disease prevention activities, and medical related transportation for the Coyote Valley Tribal Members.

 

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

Essential functions may include any of the following tasks, knowledge, skills, and other characteristics. The list that follows is not intended as a comprehensive list, but rather to provide a representative summary of the major duties and responsibilities. Incumbent(s) may or may not be required to perform all duties listed, and may be required to perform additional, position-specific tasks.

  1. Support department response to Public Health concerns and initiatives.
  2. Provide advocacy services with inter-agencies.
  3. Support clients to obtain appropriate health care and social services resources, and provide appropriate information to the respective agency.
  4. Screen and assist clients in applying for alternate financial resources.
  5. Ensures client files are kept up to date and responsible for audit compliance.
  6. Responsible for transportation of clients to appointments and doing travel paperwork.
  7. Scheduling of patient’s appointments for medical, dental, behavioral health, WIC, massage, chiropractor, foot exams, dialysis and patient referral notice appointments.
  8.  Enroll patients for various programs.
  9.  Provide monthly activity report to the Health Director.
  10. Responsible for tasks and projects as assigned by the Health Director as necessary to carry out Health, Public Health, Human Services and CHR department responsibilities.
  11. Act as mandated reporter for child abuse and adult/elder abuse.
  12. Responsible for department billing.
  13. Pick up and deliver medications to clients and keep records of such deliveries.
  14. Responsible for organizing or supporting education and outreach events.
  15. Travels as necessary to accomplish program and tribal goals and objectives.
  16.  Available to work weekends and evenings, and attend all tribal events if directed.
  17. Adheres to a strict confidentiality standard.
  18. Maintains professional behavior with positive representation of Coyote Valley while on and off the clock.
  19. Perform other duties as assigned.

 

Minimum Qualifications

To perform this job successfully, an individual must be able to perform each of the essential duties and responsibilities satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.   • High school diploma or GED.
  • Two years related work experience.
  • At least six months’ general office experience.
  • CHR Training Certificate or ability to attend and successfully complete the Basic CHR training course in the first 60 days of employment.
  •  Experience and interest in human services; especially with elders and children.
  •  Valid California Driver’s License and insurable.
  • Basic good grammar usage and spelling is required.
  • Basic computer skills including Microsoft Office and platforms such as Zoom. 

 

Skills, Knowledge and Abilities

  • Knowledge of and ability to practice HIPPA compliance 
  •  Knowledge of existing community health care services
  • Able to meet with the public and develop working relationships.
  • Able to set up and maintain filing system.
  • After three years receiving the Basic CHR training, employees are required to attend one week of CHR refresher course training.

 

Language Skills

  • Ability to communicate effectively both written and orally. 

 

 

Reasoning Ability

  • Must have the ability to deal effectively and interact well with the customers and employees.
  • Must have the ability to resolve problems/conflicts in a diplomatic and tactful manner.

 

 

Physical Demands

About the Department

Created in 1996, the department contracts directly from the Indian Health Services, Contract Health Services (CHS) under a Pub. L. 93-638 contract as well as other health and human services grants.The department supports approximately 400 eligible tribal members and residents. The department operates under a community oriented philosophy that centers on the tribal community.
