About the Department

Core Services: Clinical Access Services, Health & Protection Response, Family Health Services, Administrative
Programs: Reproductive Health, WIC, Babies First, CaCoon, Environmental Health
CHW Administrative Summary

This position directly supports the successful implementation of three of the core services offered at Crook County Health and Human Services in an administrative capacity. Indirectly, this position interacts with all CCHHS core services and programs by providing referral information and connecting inquiries with internal and external services.
This position is public facing in a receptionist capacity and requires excellent customer services and the ability to assess an individuals need and connect them with services. This position has to work within a variety of reporting systems and Electronic Health Record’s to perform chart reviews, appointment scheduling, and documentation, which includes, but is not limited to Epic; TWIST, MMIS, Ahlers, Point & Pay, OVERS and Alert IIS.
This position is considered confidential and sensitive requiring attention to detail, reliability, and the ability to work collaboratively and independently with various teams and co-workers. This position requires a self-motivated and proactive thinker who responds well to constructive criticism.
CHW Administrative Specific Education & Experience:
In addition to the education and experience requirements in the job description, it is preferred that candidates have experience with Epic or other EHR systems and customer service experience in a healthcare setting.
Summary
The community health worker position plays a key role in implementing services, projects, and programs that adhere to state and federal regulations, support public health efforts, and work alongside community partners to improve health services. As part of Crook County Health & Human Services, this position will collaborate on various public health initiatives that align with the department's mission to provide fair opportunities for those in our community to achieve their full health potential. This includes implementing data-driven, developmentally appropriate, culturally aware, trauma-informed, and sustainable approaches.

Position Duties

Essential Job Functions

Health Services Access
• Health insurance outreach, enrollment, and system navigation support for the public

• Maintain referral lists to ensure current contact information and processes

Specific Program Support:
Including but not limited to: WIC, PHEPR, Immunization, Health Promotion/Prevention Programs, Immunization, Nurse Home Visiting, Perinatal Care
Health Education
• Community outreach and program/service promotion with community members and organizations
• Support development of educational materials. Gather and review existing materials for use
• Provide the most current and accurate health topic information to clients and the general public. Information may be communicated to individuals and groups with varying ability to relate or understand

Clinical/Program Extension
• Implement care plans as written by RN or programs plans as written by Coordinators and/or other department staff
• Chart notes and billing documentation in Electronic Health Record (EHR) and/or other data systems

Program Support
• Compile data and support reporting processes and requirements
• Chart notes and billing documentation in EHR or other required systems
• Support notetaking and documentation of meetings in accordance with public meeting law and/or other established standards
• Provide input on program plan development. Support assessment and community engagement processes for plan development
• Provide input and review/editing support for grant applications and reports
• Support social media communications development

Public Health Preparedness:
• Complete Public Health Preparedness Training: ICS 100, 200, 700, and 800.
• Participate in Public Health Preparedness exercises and events.
• Deploy within the Incident Command Structure in response to public health emergencies as needed

Performance Indicators

Supervisory Responsibility
This position has no supervisory responsibilities.

Work Environment
This role will include a combination of sedentary desk work, standing presentations, and active group facilitation; some filing, lifting and carrying may be required. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, fax machines and presentation equipment – including microphones, audio and video recorders, and projectors. Presentations and events will be off-site at times and may require working in a variety of indoor and outdoor settings.
Physical Demands
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to sit; stand; walk; lift; use hands and fingers, handle or feel; and reach with hands and arms. Ability to lift up to 20 pounds, twist, and bend.
Position Type and Expected Hours of Work
This is a grant-funded full or part-time position. Typical days and hours of work are Monday through Friday, 8:00
a.m. to 5:00 p.m. Some weekends and evenings will be required for travel, events, and/or training. This position qualifies for flexible scheduling options as approved by the department Director.
Travel
Local and regional travel for presentations, events, and training can be expected. State or national travel may be an option for training and/or presentation purposes.

Minimum Qualifications

Required Education & Experience:
• High School Diploma or equivalent
• 2 years’ experience in health, nutrition, social services, customer service, and/or other related field
OR
• Any equivalent combination of training, education, and experience that provides the required skills and knowledge to perform the job
Preferred Education and Experience:
• Associates degree
• 3+ years’ experience in health, nutrition, social services, customer service, and/or other related fields
• Proficient in Spanish, ASL, or other language commonly used in local community
Additional Eligibility Requirements
• Must hold a valid Oregon driver’s license
• Able to complete a thorough employment and/or criminal history check.
• Able to successfully pass a pre-employment drug test
• Final candidates will be required to provide official proof of college degree and certified transcripts (not required for all positions).
