Tanglewood Home Care is seeking compassionate, dependable, and clinically strong Community LPNs to join our growing home care team.
This is a unique opportunity for LPNs who enjoy working with seniors, building meaningful relationships, and being part of a new and growing home care program connected to the Tanglewood campus.
As Tanglewood Home Care continues to expand, Community LPNs will support both Assisted Living residents and home care clients in the community. In the Assisted Living setting, the Community LPN will help provide scheduled nursing support for diabetic care, creams, and ordered treatments. As home care services grow, the Community LPN will also be assigned to home care cases in the community based on client needs, agency growth, and scheduling availability.

Essential Duties and Responsibilities
The Community LPN will provide nursing services under the direction of the RN Supervisor/Director of Patient Services and in accordance with physician orders, care plans, agency policy, and applicable regulations.
Responsibilities include:
Provide scheduled LPN nursing support to Assisted Living residents through Tanglewood Home Care.
Assist with morning, afternoon, and evening nursing support schedules as assigned.
Provide diabetic support, including blood glucose monitoring, diabetic observation, documentation, and reporting of abnormal findings.
Apply prescribed creams, ointments, topical treatments, and other ordered treatments within LPN scope of practice.
Provide nursing observation and report changes in condition to the RN Supervisor/Director of Patient Services.
Complete accurate and timely documentation of all care provided, treatments completed, refusals, observations, and follow-up needs.
Communicate professionally with residents, clients, families, physicians, pharmacies, Assisted Living staff, and agency leadership.
Support continuity of care between Tanglewood Home Care and the Assisted Living team.
As the home care program grows, accept assigned home care cases in the community.
Provide nursing services to home care clients in accordance with the client’s individualized plan of care.
Monitor clients and residents for changes in condition, including changes in skin, appetite, mobility, cognition, pain, behavior, or overall health status.
Assist with care coordination after hospitalizations, rehab stays, physician visits, falls, illness, or other changes in condition.
Follow infection control practices, universal precautions, and safety procedures.
Maintain confidentiality and comply with HIPAA and all agency policies.
Represent Tanglewood Home Care and Culture Care Senior Living with professionalism, compassion, and accountability.
Assisted Living Nursing Support
While working in the Assisted Living setting, the Community LPN will provide scheduled nursing support limited to services assigned through Tanglewood Home Care, including:
Diabetic support and monitoring
Blood glucose checks
Observation and reporting related to diabetic concerns
Application of prescribed creams and ointments
Ordered treatments within LPN scope of practice
Skin observation related to ordered care
Documentation of all services provided
Reporting changes in condition or concerns to the RN Supervisor/Director of Patient Services
Communication with Assisted Living leadership and care staff as appropriate
The Community LPN is not replacing the full Assisted Living care team. This position is intended to provide focused nursing support for specific ordered services while also helping build and grow the Tanglewood Home Care program.
Community Home Care Services
As Tanglewood Home Care continues to grow, the Community LPN may also provide nursing services to clients in their homes or community setting.
Community home care services may include:
Nursing observation and monitoring
Vital signs
Diabetic support
Blood glucose monitoring
Skin checks
Application of prescribed creams, ointments, and treatments
Wound care support as ordered and within LPN scope
Medication-related support as permitted by scope and agency policy
Follow-up after falls, illness, hospitalization, or change in condition
Client and family communication
Reporting concerns to the RN Supervisor/Director of Patient Services
Documentation of all care and services provided
Assignments may vary based on client needs, agency growth, scheduling needs, and the LPN’s availability and qualifications.
Schedule
Community LPNs may be assigned to scheduled nursing coverage, including morning, afternoon, evening, weekday, and/or weekend shifts.
Initial scheduling may include regular Assisted Living nursing support for diabetic care, creams, and treatments. As home care cases are added, schedules may include a combination of Assisted Living support and community home care visits.
Qualifications
Current New York State LPN license in good standing.
Prior experience in home care, assisted living, long-term care, or senior care preferred.
Strong clinical observation and documentation skills.
Ability to work independently and communicate effectively.
Compassionate, dependable, organized, and client-focused.
Must be able to follow physician/provider orders, care plans, and agency policy.
Must maintain required health records, background checks, and annual in-services.
Valid driver’s license and reliable transportation preferred, especially for community home care assignments.
