Community Vision is looking for a Direct Support Professional to join our Community Living team. The Community Living Specialist works one-on-one with multiple individuals receiving our services, providing direct support and facilitating various activities to ensure the individuals receiving our services have opportunities at home and in the community that create an optimum range of choice, respect, competency, community presence and relationships.
Key Job Functions of the CL Specialist
The Community Living Specialist works with individuals and their support team in identifying and monitoring specific activities to achieve goals in their Individual Support Plan, according to the Service Agreement. They provide individualized direct support, which may include, but is not limited to:
- financial management,
- medical management,
- grocery shopping,
- community connections,
- household maintenance, and
- creating and maintaining relationships through social skill development.
Schedule: M-F 40 hours/week; flexible depending on support needs of individuals
Pay: $23.00/hourly; non-exempt
Mileage: $.725/mile reimbursed for qualified trips

Requirements
The ideal candidate for the Community Living Specialist role is someone who is highly organized, adaptable, and has the ability to quickly establish healthy connections with the individuals receiving our services.
Additionally, the Community Living Specialist must:
- Frequent, daily travel is required.* You must be available/able to drive to various locations in the Portland Metro Area and be willing to transport Supported Individuals in your own vehicle. (Mileage reimbursement rate is $.725/mile)
- Must possess current liability insurance and have a valid Driver’s License.
- Be able to push, pull, and lift up to 50 lbs.
- Have at least one-year paid experience in developmental disabilities field, or in human services.
- Obtain Approved Criminal History Check through the Oregon Department of Human Services Background Check Unit.
- Not be on the OIG List of Excluded Individuals and Entities
*Transporting Supported Individuals during a shift and driving from one shift to the next (between Supported Individuals) is a significant part of the day-to-day job. If driving is something you do not want to do, you may want to reconsider applying to this job. While we strive to create efficient, regular schedules, driving has been noted as one of the primary drawbacks of this job in exit interviews by previous CL Specialists.
