About NACC

The Native American Community Clinic is an urban Federally Qualified Health Center (FQHC) dedicated to providing culturally tailored, whole-person care to the Native American community in Minneapolis. NACC integrates traditional healing values with evidence-based medicine to address health disparities and promote wellness across generations.

Role

NACC is seeking a Family Nurse Practitioner to deliver high-quality, culturally responsive primary care to patients of all ages. Working within an integrated care team, the Family Nurse Practitioner will provide relationship-based care across the lifespan — including acute, chronic, preventive, and wellness services — while contributing to NACC's mission of health equity and community wellness.

Responsibilities 

  • Provide comprehensive primary care across the lifespan, managing conditions including diabetes, hypertension, behavioral health concerns, substance use disorders, and maternal/child health needs
  • Conduct patient assessments, formulate diagnoses, develop individualized treatment plans, and prescribe medications within scope of practice
  • Deliver care grounded in cultural humility and responsive to the traditions and strengths of the Native American community
  • Collaborate with an interdisciplinary team including medical and behavioral health providers, traditional health programming, care coordinators, and community partner
  • Participate in quality improvement initiatives, care conferences, and program development
  • Maintain accurate, timely documentation in the electronic medical record
  • Participate in Saturday clinic coverage on a rotating basis
  • Share in on-call coverage as assigned

Job Requirements 

Required:

  • Current, unrestricted RN and APRN licenses from the Minnesota Board of Nursing
  • Certified Family Nurse Practitioner from national accrediting body (AANP or ANCC)
  • DEA registration (or eligibility to obtain)
  • BLS/ACLS certification (or willingness to obtain prior to start)

Preferred:

  • Experience working in community health or an FQHC setting
  • Experience working with Native American / American Indian communities

Physical Requirements

This role requires the ability to use hands for fine motor tasks, communicate verbally and auditorily, walk frequently, and occasionally stand, sit, reach, stoop, or kneel. Must be able to lift up to 10 lbs frequently and up to 25 lbs occasionally. Reasonable accommodation may be provided to enable individuals with disabilities to perform essential functions.
