Fellowship Program 

This opening is Advanced Practice Provider Fellowship Program eligible! Community Health Network has developed a holistic program to assist newly graduated Advanced Practice Provider. The goal of this intentional and systematic training program is to support the transition of new providers from academia into clinical practice and prepare them for long careers within Community Health Network. Your journey with the Advanced Practice Provider Fellowship Program begins once you have accepted a role with our Network.

We are seeking an Advanced Practice Provider to join one of the nation’s most integrated healthcare systems, with over 1,300 providers in Indianapolis, IN. This opportunity is located at County Line Family Medicine Practice, at 333 East County Line Road, Suite B Greenwood, IN 46143.

 

Highlights

Community Health Network has an excellent opportunity for a family medicine trained Advanced Practice Provider to work at our Family Medicine Care practice located in Greenwood.

 

  • Single, robust electronic medical record system (EPIC)

Experience

  • Must successfully complete a nationally recognized Advanced Practice Nurse certification examination or have current national certification; must maintain certification on an ongoing basis.
  • Must be eligible for prescriptive authority under the criteria outlined by the Indiana State Board of Nursing; must maintain eligibility for prescriptive authority on an ongoing basis.
