Job Description

Piedmont Health Partners is a local Clinically Integrated Network owned by AnMed Health and comprised of employed and independent health care providers. The role of the Team assistant establishes and maintains collaborative relationships with Health Partner teammates, physicians, other health care providers, patients and their families, to achieve desired patient outcomes throughout the continuum of care. The goal is to help PHP achieve the “Quadruple Aim” of lowering health care costs, improving patient satisfaction, increasing quality of care and enhancing the lives of healthcare providers in the network.  



The Team Assistant is the first responder to PHP helpline calls. The Team Assistant is responsible for providing support with clerical needs, scheduling and administrative support necessary to assure delivery of clinical care for the patient and staff of a given interdisciplinary group, coordination of the paper flow from the teams in the group, data entry of documentation and communication with patients, physicians, and staff.



Qualifications: Current CMA or LPN required, Minimum 3 years of experience (can be combination of clinical experience). Willingness to obtain Certified Health Coach status. Have a current South Carolina motor vehicle license. Comfortable in a variety of settings meeting patients and working with a team. Willing to use personal transportation (mileage reimbursed) to do home visits and other patient centered travel such as joint physician visits. Familiarity with EHR, Microsoft word and/or other similar computer software. Willingness to learn new skills and function in a non-traditional role.
