Job Title:  Home Health Aide & Office Assistant

Supervisor: Home Care Director

FLSA Status: Non-exempt, Full Time

 

 

Job Summary 

Under the direct supervision of a Registered Nurse (RN) and the Home Care Director, this hybrid role splits time equally (50/50) unless otherwise assigned between direct field care and office assistant. The employee provides essential home health care services to PACE participants while simultaneously executing administrative tasks to support the PACE of the Triad Home Care office operations. All duties must adhere to PACE guidelines, training protocols, and organizational policies

Requirements

Specifications 

Education, Training, and Experience

  • High School Degree: Or an equivalent GED certificate.
  • Active CNA I Listing: Currently listed as a Nursing Assistant (NA) I with the North Carolina Department of Health and Human Services (NC DHHS).
  • Clean Registry Status: No substantiated findings of neglect, abuse, or misappropriation of resident property.
  • Approved Certification: Certificate from an approved CNA training program.
  • Elderly Care Experience: At least 1 year of experience working with an aging, frail elderly population.
  • Clinical Experience: At least 6 months of clinical experience as a Nursing Technician, Nursing Assistant, Corpsman, or EMT.
  • Transportation: Valid North Carolina driver’s license and an insured, reliable vehicle.

Tech and Equipment Competencies

  • Electronic Health Records: Experience entering clinical data into EHR / EMR platforms.
  • Scheduling Software: Ability to navigate, view, and update care schedules in digital platforms.
  • Office Equipment: Proficient in operating multi-line phone systems, commercial scanners, printers, and copiers.
  • Core Applications: Functional knowledge of Microsoft Office Suite (Word, Excel, Outlook) and email communications.

Knowledge, Skills, and Abilities

  • Teamwork: Proven ability to work effectively within an Interdisciplinary Team (IDT) setting.
  • Scope of Practice: Strict adherence to the authorized scope of practice and PACE competencies approved by CMS.
  • Language Skills: Ability to read, write, speak, and communicate effectively in English.
  • Sensory Skills: Ability to clearly see medical records, communicate via phone, and hear blood pressures.

Physical Requirements (With or Without Reasonable Accommodations)

  • Mobility: Ability to bend, stoop frequently, climb stairs, and walk distances independently.
  • Lifting Capacity: Ability to lift, position, and transport participants and durable medical equipment weighing at least 50 pounds.
  • Body Mechanics: Constant utilization of correct body mechanics during transfers, lifts, and positioning.
  • Dexterity: Ability to perform repetitive hand and wrist movements.

Summary

Key Responsibilities  

Home Health Aide Responsibilities (50% Field Time)

  • Assist with ADLs: Provide basic personal care, hygiene, and assistance with activities of daily living to assigned participants.
  • Support therapies and meals: Deliver care across settings including nursing assistance, restorative therapies, activities, and meals.
  • Document clinical observations: Record observations, treatments, procedures, and participant activities accurately and timely.
  • Collaborate with the IDT: Coordinate 24-hour care delivery with members of the Interdisciplinary Team (IDT).
  • Report on health status changes: Communicate participant condition changes immediately to the RN and other IDT members.
  • Maintain safety standards: Protect PACE of the Triad by adhering to professional standards and federal, state, and local requirements.

Administrative Assistant Responsibilities (50% Office Time)

  • Manage office communications: Answer phone calls, route inquiries, and greet visitors at the front desk.
  • Coordinate team scheduling: Assist leadership in scheduling home care staff to ensure full coverage for participants.
  • Maintain digital records: Enter data and update electronic health records, caregiver documentation, and office files.
  • Process visit documentation: Review timesheets in scheduling software for accuracy.
  • Provide clerical support: Draft memos, print resources, file documents, and assist with meeting preparations.

Manage office inventory: Track, order, and organize clinic and office supplies

 

Environmental Factors 

Tasks performed during the field rotation have a high potential for exposure to bloodborne pathogens. Employees must demonstrate strict adherence to safety protocols during:

  1. Assisting with mouth care.
  2. Assisting with care of non-intact skin.
  3. Assisting incontinent participants with personal care.
  4. Administering enemas.
  5. Shaving participants.

Demonstrates knowledge of and supports PACE mission, vision, values, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.
