Home Health LPN

Guthrie, OK (and surrounding areas)

What You’ll Do
  • Perform comprehensive assessments and document patient condition at every visit
  • Monitor vital signs, review medications, and complete evaluations
  • Administer physician-ordered medications safely and accurately
  • Provide skilled wound care, dressing changes, and track healing progress
  • Educate patients and families on home care techniques, medication management, and health strategies
  • Collaborate with the care team to deliver personalized, high-quality support
What We’re Looking For
  • Graduate of an accredited nursing program
  • Current Oklahoma LPN license (required)
  • Current CPR certification (required)
  • 2+ years of home health, palliative care, or related experience (preferred)
  • Strong communication skills and a genuine passion for patient-centered care
  • Valid driver’s license and willingness to travel to patient homes in the area
  • Company car provided for all work-related travel
