Essential Job Functions/Responsibilities

Referral Coordination, Admissions & Explanation of Benefits (EOB):

• Meet with patients and families to explain hospice benefits and coverage, ensuring clear understanding of hospice philosophy and available services.
• Relay all information gathered during EOB meetings to the Intake and Clinical teams promptly and accurately to support a smooth admission process.
• Request, verify, and organize all documentation required for admissions.
• Address questions regarding benefits eligibility as directed by the Intake/Authorization team.
• Collaborate closely with Community Liaisons to ensure seamless referral transitions and strengthen community relationships.

Patient Care & Symptom Management:

• Provide direct skilled nursing care under RN oversight, per the State Nurse Practice Act.
• Implement and document the care plan, including medication management, symptom monitoring, and equipment use.
• Evaluate patient response to care and report changes to the clinical team
Interdisciplinary & Community Engagement.
• Assist clinical team with ensuring documentation in facility (ALF/RCFE/SNF) binders are up to date and accurate.
• Build positive relationships with facility staff and represent Acacia hospice services.

Documentation & Compliance:

• Ensure accurate, timely documentation (visit notes, referrals, EOB explanations.)
• Adhere to hospice policies, quality standards, and participate in performance improvement processes.

Additional Responsibilities:

• Assist with admissions, discharges, and other administrative tasks.
• Perform as guided by the Administrator and / or Director of Patient Care Services.

Service Coverage Area: Inland Empire

Position Qualifications:
• California LVN License
• Valid California Driver’s License
• American Heart Association Basic Life Support (BLS) certification
• 2 years of hospice work experience
