Registered Nurse After Hours
Salary: $80-89k/yr BOE
Schedule: Full-time | Thursday- Saturday
Shift Details: 5:00 PM- 8:00 AM
Location: Franklin/Middletown/Monroe, Ohio
Requirements of the Hospice RN role:
Licensed RN in state of practice
CPR/BLS Certified
A minimum of 1 year of Clinical experience
Valid Drivers License and Auto insurance Coverage
Reliable transportation: Willing to travel
On-call/ triage experience preferred
About the RN role:
Provide skilled nursing care to patients receiving end-of-life (EOL) care in facility and home settings.
Respond to phone calls from patients, families, caregivers, and facilities regarding symptom concerns, changes in condition, and emergent situations.
Conduct home and facility visits to assess and manage patients experiencing critical symptoms, including pain, respiratory distress, agitation, anxiety, and emotional crises.
Provide crisis nursing interventions and urgent symptom management to address acute changes in patient condition and support patient comfort.
Conduct death visits, complete required documentation, and provide support and guidance to families and caregivers following a patient's death.
Complete after-hours admissions, including comprehensive assessments, initiation of the plan of care, and coordination with the interdisciplinary team.
Facilitate hospital discharges and transitions of care by coordinating admissions, ensuring continuity of services, and communicating patient needs to the care team.
Assist the RN Case Manager (RNCM) in implementing and carrying out the patient's plan of care.
Assist the nursing team in performing specialized procedures, as defined by the state Nurse Practice Act, under the direction of the RNCM and physician orders.
Monitor patient status and promptly communicate significant changes, concerns, and observations to the RNCM.
Respond to facility escalations by assessing patient needs, implementing appropriate interventions, and collaborating with facility staff, physicians, and the hospice team.
Provide nursing support for neighboring location overflow, including patient visits, admissions, and urgent care needs as assigned.
Actively participate in Interdisciplinary Group (IDG) meetings to collaborate on patient care plans, ensure plans remain current, and address identified patient and family needs.
Reinforce patient and family education regarding disease progression, symptom management, hospice philosophy, and end-of-life care.
Maintain accurate, timely, and compliant clinical documentation in accordance with organizational, regulatory, and accreditation standards.
Demonstrate accountability for professional growth and development through continuing education, workshops, conferences, in-service training, professional organizations, independent research, and self-directed learning.
Collaborate effectively with patients, families, caregivers, physicians, facilities, and members of the interdisciplinary team to ensure the highest quality of patient-centered care.
