Essential Job Functions/Responsibilities:
• Conduct all General Inpatient (GIP) visits in the hospital’s GIP unit, ensuring compliance with hospice standards and patient care protocols.
• Serve as a clinical resource between hospital staff, hospice team, patients, and families.
• Perform admissions in inpatient facilities and other settings as needed.
• Assist with overflow clinical needs as required, including home visits or other patient care tasks.
• Provide Explanation of Benefits (EOB) to patients & families, ensuring clarity on hospice services and coverage.
• Educate hospice philosophy & services with interested non-admitted patients who request to learn about hospice.
• Collaborate with interdisciplinary team members to develop and update individualized care plans.
• Documentation submitted upon completing each visit or latest by the end of the day.
• Perform as guided by the Administrator and / or Director of Patient Care Services.
Documentation & Compliance:
• Ensure accurate, timely documentation (visit notes, referrals, EOB explanations.)
• Adhere to hospice policies, quality standards, and participate in performance improvement processes.
Service Coverage Area: Low Desert /Palm Desert
Shift: 12p-8p, Weekends and Weekdays availability required
Position Qualifications:
• California RN License
• Valid California Driver’s License
• American Heart Association Basic Life Support (BLS) certification
• 2 years of hospice work experience
