The Intake Liaison is responsible for coordinating the referral-to-admission process for home health patients in accordance with physician orders, agency policy, and regulatory and accreditation requirements.
Responsibilities:
- Support the delivery of safe, compliant, and goal-oriented patient care.
- Promote positive relationships with patients, families, referral sources, and interdisciplinary team members.
- Review referrals to ensure receipt of acceptable physician orders and complete documentation.
- Obtain and document initial authorizations for all payor sources, including Medicare Advantage, Managed Medicaid, VA, and commercial insurance plans, ensuring timely approval for services.
- Verify, negotiate, and document insurance and payer coverage for all referral sources.
- Facilitate communication between physicians, office staff, and field clinicians in accordance with established chain of command.
- Partner with the Business Development team to ensure timely follow-up on referral inquiries, accurate tracking of referral outcomes, and strong conversion of referrals to admissions.
- Utilize SmartSheets, EMR, and related agency systems to track referral progress, authorization status, and admission readiness.
Qualifications:
- Associate degree in healthcare, business, or related field preferred.
- Minimum of one (1) year of experience in healthcare intake, authorization coordination, or home health preferred.
- Knowledge of Medicare, Medicaid, and commercial insurance authorization processes strongly preferred.
- Proficiency in EMR systems, SmartSheets, and Microsoft Office Suite.
- Strong interpersonal, organizational, and communication skills.
