Position Summary
Hamilton-Madison House’s Caregiver Support Program serves informal/unpaid caregivers of older adults, and their loved ones, throughout Manhattan’s community districts 1-7, comprising all of Manhattan below 59th Street and the west side of Manhattan to the northern tip of central park.
This position primarily is that of the Intake Coordinator for the program. They will be responsible for taking all program intake calls and referrals ensuring that all potential callers feel engaged, and that trust and rapport is built as the first door to HMH Caregiver Program. Complete thorough intakes that clearly document the best of the initial connection, the caregiver’s eligibility and presenting problem. The position is the key to the caller with a positive impression of the HMH Caregiver program and services available.
This position may be required to carry a few Caregiver clients as needed arises. Care Managers conduct outreach, telephone intakes and in-person assessments and reassessments, counseling, screen clients for eligibility and assist them in applying for benefits and entitlements, provide information and referral services, develop and implement client care plans, including Respite and Supplemental Services and coordinate or provide caregiver training, and support groups.
Responsibilities
Intake Coordination
- Establish rapport and convey an understanding of and empathy for client concerns.
- Demonstrate Engagement and Active Listening.
- Through engagement elicits the care givers presenting problems.
- Educate the caller to caregiver services and potentially help the caller to understand that they are a Caregiver.
- Determine program eligibility.
- Provide any relevant information and resources
- Provide a thorough description, if possible, of the caregiver and care receiver’s relationship, identify caregivers current support to the care receiver and determine IADL and ADL needs of the care receiver.
- Through building a trusting relationship attempting, if possible, to obtain care giver consent to enroll in the program.
- Also be prepared to share with the client the ArchAngel questionnaire, also a tool to determine caregiver stress and the beginning of a care plan.
- If not in programs service area or needs not related to caregiver services; make necessary referrals to resources/services.
- Assure the client, if eligible or interested in caregiver services that they would be hearing from an assigned care manager
- Coordinate with the Assistant Director by sharing thorough description of the caregiver and the caregiver needs so they might assign an appropriate care manager
Care Manager Role
- Maintain small client caseload of ongoing and short-term clients/caregivers.
- Conduct telephone intake interviews.
- Conduct home visits to assess caregiver and care recipient needs using standard assessment forms and documentation of ADL and IDL needs.
- Use standard screening tools to assess clients for benefits/entitlements eligibility and assist them in completing and submitting forms and following up to ensure benefits/entitlements are received.
- Utilize support care planning for the caregiver and coordinate referrals to appropriate resources.
- Maintain case records on database including case notes and documentation of services provided
- Ensure timely follow-up on caregiver contacts and assessments and reassessments.
- Participate in interdisciplinary team case conferences and review and implement programming to benefit the Hamilton-Madison House’s overall caregiver services department
- Other duties as assigned
Minimum Qualifications and Requirements
Education & Experience
- Option 1: (Preferred) Master’s Degree in Social Work (MSW) or a related field + 1 year of casework/counseling experience (MSW internship counts)
- Option 2: Bachelor’s Degree in Social Work (BSW) or a related field + 2 years of care management/counseling experience (internship counts).
- Asset: Prior experience working in intake processes is highly preferred.
- Aging & Disability: Knowledge of the biological, psychological, and social aspects of aging, and the impact of illness on families.
- Benefits & Entitlements: Familiarity with senior benefits systems and direct experience completing benefit application forms.
- Clinical Skills: Strong grasp of client engagement, comprehensive assessment, and crisis de-escalation.

- Communication: Exceptional verbal, written, and telephone communication skills.

- Software: Basic computer literacy in Microsoft Windows and MS Office (Word, Excel, Outlook).
- Data: Familiarity with client databases; specific knowledge of Vive software is a preferred asset.
- Teamwork: Proven ability to work both independently and within interdisciplinary teams.
- Fieldwork: Willingness to take initiative and conduct frequent community and home visits.
- Mobility: Ability to travel via public transportation and climb stairs to access client homes and run info sessions.
- Bilingual: High preference for candidates fluent in English/Cantonese, English/Spanish, English/French Creole, or English paired with another community language.
Rate Range: $32.95 – $35.70 per hour (Equivalent to $60,000 – $65,000 annually, annualized at 1,820 hours).
Hours: Monday to Friday, 9:00AM to 5:00PM, 35 hours per week
