Blize Healthcare is seeking a detail-oriented and compassionate Intake Coordinator to join our team. This role is essential in ensuring patients experience a smooth, supportive transition into home health and hospice care.

Responsibilities:
  • Manage daily patient referral and intake operations.
  • Verify insurance coverage and assist the billing department with claim processing.
  • Complete insurance forms and respond to patient inquiries.
  • Schedule home health and hospice visits efficiently.
  • Answer, screen, and direct phone calls; take messages; schedule appointments.
  • Greet and assist visitors in a professional manner.
  • Receive, sort, and distribute mail, documents, and packages.
  • Maintain medical supply inventory.
  • Select and package DME supplies.
  • Support colleagues with general administrative tasks.
  • Maintain Patient Record.
  • Send Orders needed to sign to physicians and follow up with physicians.
  • Attach patient records and consent forms in the company system.

Requirements:
  • Minimum 1 year of experience in the medical field.
  • Strong verbal communication skills.
  • Excellent interpersonal and customer service skills.
  • Basic understanding of administrative and clerical procedures.
  • Proficiency in Microsoft Office (Word, Excel, Outlook) and Google Workspace (Docs, Sheets).
