As a Primary Care Provider, you will provide primary care to patients in long-term care facilities.
This includes:
- Developing and implementing personalized treatment plans based on patient needs
- Offering medication management, including prescribing and monitoring medications
- Collaborating with a multidisciplinary team of healthcare professionals to ensure patient care
- Maintaining accurate medical documentation in line with regulatory standards
- Providing education and support to patients and their families regarding mental health conditions and treatments
Qualifications:
- Current Nurse Practitioner license in specified location
- At least two years of experience is required
- Experience providing care to geriatric populations in long-term care settings is preferred but not required
- Excellent communication skills, especially when working with facility staff and residents' families
- Ability to manage time effectively, including flexibility in scheduling and being responsive to patient needs
