Marketing Liaison
Join Blessed Home Health, a dedicated healthcare organization, as a Home Health Care Liaison in Oregon, Ohio. As a Marketing Liaison, you will serve as a vital bridge between patients, families, and healthcare providers to ensure seamless delivery of personalized home health services.
Responsibilities:
Act as the primary point of contact for patients and their families, providing clear information about available home health services and answering inquiries with enthusiasm and professionalism
Collaborate with healthcare providers, case managers, and community organizations to develop and implement tailored care plans that meet individual patient needs
Conduct patient assessments including observation of vital signs, assistance with activities of daily living (ADLs), and monitoring for changes in condition such as dementia or Alzheimer's disease
Assist with documentation using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to ensure accurate medical records, medication administration logs, and medical documentation are maintained in compliance with HIPAA regulations
Coordinate scheduling for home visits, hospice care, long-term care, or live-in employment arrangements while ensuring all safety protocols are followed
Educate patients and caregivers on medication management, catheter care, dementia or memory care strategies, and other specialized home health techniques such as Hoyer lift use or CPR procedures
Promote safety by observing patient conditions, assisting with mobility using equipment like Hoyer lifts, and ensuring a clean environment through light cleaning and laundry when necessary
Requirements:
Strong knowledge of medical terminology and familiarity with EMR/EHR systems for accurate record-keeping
Experience working with individuals with developmental disabilities, dementia, Alzheimer’s disease, or in hospice settings
Proficiency in patient observation skills including vital signs monitoring and assessment of health status changes
Skilled in assisting with ADLs such as bathing, dressing, meal preparation, and mobility support using assistive devices like Hoyer lifts
Ability to communicate effectively with diverse populations including seniors, individuals with disabilities, or those in assisted living environments
Knowledge of HIPAA compliance standards to ensure confidentiality of medical records and patient information
Certifications such as CPR or first aid are highly desirable; experience in long-term care or home & community care is preferred
Capable of working independently while managing multiple patient cases with compassion and professionalism
