Job Description

 POSITION SUMMARY

The position provides comprehensive, patient-centered care across various clinical settings, ensuring the highest standard of medical service for our community. The Nurse Practitioner will assess, diagnose, treat, and manage both acute and chronic health conditions, working closely with physicians, specialists, and other healthcare providers to deliver quality care. This position offers the opportunity to work in a collaborative, team-oriented environment with a focus on promoting health, improving patient outcomes, and delivering compassionate care.

CORE FUNCTIONS

1. Conduct thorough patient assessments, including taking medical histories, performing physical exams, and ordering and interpreting diagnostic tests. 

2. Diagnose and manage a wide range of health conditions, both acute and chronic, and prescribe medications as appropriate. 

3. Provide patient education on disease prevention, health promotion, and the management of medical conditions. 

4. Collaborate with physicians, specialists, and interdisciplinary teams to develop and implement individualized care plans. 

5. Monitor patient progress, adjust treatment plans as necessary, and ensure appropriate follow-up care. 

6. Document patient interactions, assessments, and treatments in compliance with hospital policies and state regulations. 

7. Participate in patient care coordination, including referrals to specialists and follow-up appointments. 

8. Engage in continuous professional development, staying current on advancements in healthcare, clinical best practices, and healthcare regulations. 

9. Participate in quality improvement initiatives to enhance patient care and contribute to the overall mission of La Paz Regional Hospital and Clinics. 

10. Provide guidance and mentorship to other healthcare staff as needed. 

PHYSICAL DEMANDS/ENVIRONMENT FACTORS

Able to stand, walk, bend, squat, reach, and stretch frequently. Possess physical agility and adequate reaction time to respond quickly and appropriately to department needs. Needs adequate hearing and visual acuity, including adequate color vision. 

Requires fine motor skills, adequate eye-hand coordination, and ability to grasp and handle objects. May be required to lift up to 50 pounds. Must use standard precautions due to threat of exposure to blood and bodily fluids. Needs ability to communicate effectively through reading, writing, and speaking in person or on telephone. May require periodic use of personal computer.
