Nurse Practitioner – New Participant Assessment 

  • Location: Sacramento, CA
  • Compensation: $150,000 - $180,000 *Negotiabe 
  • Schedule: Monday – Friday, 8:00 AM – 5:00 PM
  • Work-Life Balance | No Nights, Weekends, or Holidays Except for Minimal, Phone-Based On-Call 
  • Light patient volume

Requirements: Nurse Practitioner – New Participant Assessment: 

  • Current CA State issued Nurse Practitioner license 
  • Current active DEA license
  • 1+ year as a practicing Nurse Practitioner 
  • 5+ years a Registered Nurse 
  • Experience in elderly care/geriatric setting

Overview of Responsibilities:

The Nurse Practitioner – New Participant Assessment specializes in comprehensive evaluations and care planning for new participants. This role conducts initial assessments, formulates treatment plans, and collaborates closely with the interdisciplinary team to optimize participant health outcomes. 

  • Perform detailed physical, functional, and psychosocial assessments for new participants.  
  • Order and interpret diagnostic tests. 
  • Develop and update individualized treatment plans, including establishing goals of care as well as prescribing medications and therapies.  
  • Provide education to participants, caregivers, and staff regarding health conditions and treatments.  
  • Participate in interdisciplinary team meetings and family conferences to align care plans.  
  • Ensure accurate and timely documentation of all assessments and interventions.  
  • Manage acute and chronic conditions in collaboration with other providers across home, clinic, and facility settings.  
  • Other duties as assigned.
