Nurse Practitioner (NP) or Physician Assistant (PA) - Geriatrics/Internal Medicine

Job Summary:

Carease Health - NJ is seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a hybrid role, this position combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. Our company prioritizes proactive measures for enhanced physical health and well-being, incorporating Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) into our comprehensive preventative care program.

Responsibilities:

  • Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work from home

  • Conduct assessments, develop treatment plans, and prescribe medications as needed

  • Collaborate with healthcare teams to ensure comprehensive care and minimize hospitalizations

  • Utilize Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to enhance patient outcomes

  • Maintain accurate and timely documentation of patient care and outcomes

  • Participate in quality improvement initiatives to enhance patient care standards

Requirements:

  • Active NP or PA license (in the state of practice)

  • 1+ year of experience in Geriatric care and documentation review

  • Excellent communication, collaboration, and teamwork skills
