Nurse Practitioner

Job Summary:

We are seeking an experienced Nurse Practitioner (NP) with a background in Geriatrics or Internal Medicine to join our team at Carease Health - OH in Columbus, OH. As a Nurse Practitioner, you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

• Provide high-quality preventative care to residents in skilled nursing facilities

• Conduct on-site visits and remote work from home, supported by a dedicated scribe team

• Collaborate with the facility and healthcare team to develop and implement individualized care plans

• Build strong relationships with residents, families, and facility staff

• Document patient progress and communicate with healthcare team

• Participate in quality improvement initiatives to reduce hospitalizations

• Stay up-to-date with industry developments and best practices in Geriatrics and Internal Medicine

Requirements:

• Active NP license (in the state of practice)

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills

• Masters' level education in Nursing
