We're seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

• Provide high-quality preventative care to residents in skilled nursing facilities

• Conduct on-site visits and remote work from home

• Collaborate with the facility team to ensure seamless integration of services

• Build strong relationships with patients, families, and facility staff

• Maintain accurate and timely documentation of patient care

Requirements:

• Active NP or PA license (in the state of practice)

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
