Job Summary

We are seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. This hybrid role combines on-site visits with remote work from home, supported by a dedicated scribe team for efficient documentation. As a Nurse Practitioner at Carease Health - IL, you will build strong relationships with the facility, maintain a patient-centered approach, and be a collaborative team player.

Responsibilities

  • Provide high-quality preventative care to residents in skilled nursing facilities through on-site visits and remote work

  • Utilize Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Collaborative Care Management (CoCM) to proactively monitor vital signs and detect abnormalities

  • Collaborate with a dedicated scribe team for efficient documentation

  • Build strong relationships with the facility and maintain a patient-centered approach

  • Be a collaborative team player, working closely with other healthcare professionals to ensure seamless care delivery

Requirements

  • Active NP or PA license (in the state of practice)

  • 1+ year of experience in Geriatric care and documentation review

  • Excellent communication, collaboration, and teamwork skills
