Yavapai County Community Health Services is seeking a dedicated Nurse Practitioner to provide high-quality, compassionate care in a collaborative primary care setting. The ideal candidate will have experience in OB-GYN or reproductive health and a passion for serving diverse populations. Bilingual in Spanish skills and knowledge of or experience working in a Federally Qualified Health Center (FQHC) environment are highly preferred. - Provides comprehensive medical care in accordance with regulations and scope of practice for licensure in Arizona, State, Federal and other grant requirements, and the requirements of all payors.
- Examines and assesses patients; establishes diagnoses; conducts appropriate diagnostic tests; develops implements and evaluates treatment plans.
- Refers patients to collaborating physicians, other specialists, and other medical facilities as needed. Provides patient education as needed.
- Documents medical visit in the electronic health records system in a timely manner.
- Coordinates patient care with the clinic support staff.
- Identifies potential opportunities to improve services, suggests solutions and new innovations and participates in their implementation.
- Participates in the Quality Improvement Program, as requested.
- Represents the department in community outreach events such as health screenings/physicals at Health Fairs, school sports physicals and community sports physicals.
- Performs other job-related duties as assigned.
Minimum Qualifications
Education:
Master’s Degree in Nursing and relevant Nurse Practitioner Certification in the specialty area for the position held.
Experience:
No experience necessary.
Licenses or Certifications:
Possession of a current license to practice as a registered nurse in the State of Arizona. Current Arizona DEA Certificate, Arizona Prescribing Authority Certificate and Basic Life Support.
Additional Requirements:
Must possess a valid Arizona driver’s license.
Must possess a current fingerprint clearance card issued by the Arizona Department of Public Safety
