Vacancy Information:
The Department of Homeless Services and Housing (HSH) is seeking a well-qualified and highly motivated individual to fill a vacancy in the Clinical Services area. This transfer opportunity is located at the HSH Maple Office (655 Maple Avenue, Los Angeles, CA 90014), and the Department currently offers a 9/80 work schedule, as well as a hybrid model that allows for a combination of telework and onsite presence.Essential Job Functions
Duties include, but are not limited to:
- Obtains medical, psychiatric, and substance use disorder (SUD) histories from clients and conducts comprehensive assessments to diagnose and treat medical and behavioral health conditions. Evaluates client need for psychotropic and medication-assisted treatment (MAT); furnishes and administers medications within scope of practice; monitors client response to treatment; and orders, reviews, and interprets laboratory tests. Provides clinical services in unsheltered and sheltered settings, including interim housing, permanent supportive housing, and enhanced residential care facilities.
- Leads Building Response Team (BRT) efforts by identifying facilities requiring enhanced clinical and environmental intervention and delivering intensive services to clients and supporting front-line case managers. Provides direct clinical care, coordinates services for complex clients, and delivers training and technical assistance to Intensive Case Management Services (ICMS) and mental health staff to strengthen care delivery.
- Provides referrals to psychiatric and mental health services, including psychotherapy, psychosocial rehabilitation, and recovery-oriented care. Provides SUD counseling and referrals to appropriate treatment services. Documents treatment goals, interventions, clinical outcomes, and medication/MAT effectiveness in client health records and treatment plans.
- Provides crisis intervention and supportive counseling and completes 72-hour holds for evaluation of involuntary treatment, as appropriate within scope of practice.
- Participates in quality improvement and clinical risk management activities, including development and review of policies, procedures, and protocols. Attends clinical case conferences and contributes to data collection and analysis to support system-wide quality improvement initiatives.
- Supports RN complex care managers and ICMS providers in delivering care to high-need, high-cost clients within the Permanent Supportive Housing (PSH) program to meet Enhanced Care Management (ECM) standards under CALAIM.
- Participates as an active member of a multidisciplinary treatment team, providing consultation and clinical guidance to both clinical and non-clinical staff, including case managers and outreach workers, in areas such as diagnosis, treatment planning, and recovery strategies.
- Participates in staff education through development and delivery of training and joint educational presentations to enhance clinical knowledge and service delivery across disciplines.
- Provides clinical support to interim housing (IH) and enhanced residential care (ERC) nursing staff in triaging clients for placement into housing and care facilities.
- Participates in clinical quality assurance activities for IH and ERC programs as directed by the Medical Director.
Requirements
Qualifying Requirements:
Permanent County of Los Angeles employees who have successfully passed their initial probationary period and hold the payroll title of a Nurse Practitioner.
Additional Information
Desirable Qualifications:
- Experience providing healthcare services to people experiencing homelessness (PEH).
- Experience delivering field-based or street medicine clinical care services.
- Familiarity with nurse-led complex care management services for high-need, high-cost Medi-Cal beneficiaries.
- Demonstrated ability to lead teams in performance improvement and quality enhancement initiatives.
- Demonstrated ability to effectively teach and communicate with both clinical and non-clinical staff.
- Experience and expertise in assessing and managing individuals with SUD, including knowledge of MAT and addiction psychiatry.
- Experience and expertise in assessing and managing individuals with serious and persistent mental illness.
- Experience and expertise in trauma-informed care and harm reduction principles.
- Familiarity with Los Angeles County Department of Mental Health (DMH), Los Angeles County Department of Public Health (DPH), and community-based mental health and substance use programs and resources.
