Job Description

We are currently seeking a skilled Nurse Practitioner to join our dynamic team. The Nurse Practitioner (NP) will play a pivotal role in assessing, diagnosing, and managing the healthcare needs of our clients. This position is essential for bridging the gap between patient needs and clinical services in a supportive and collaborative environment.

The Nurse Practitioner will conduct comprehensive health assessments, develop treatment plans, and provide ongoing monitoring of chronic and acute conditions. The ideal candidate should possess a deep understanding of behavioral health and be passionate about improving the quality of life for those we serve.

Key Responsibilities:

- Perform thorough physical examinations and health assessments for participants.

- Develop and implement individualized care plans based on assessment findings and client needs.

- Diagnose and manage acute and chronic health conditions.

- Prescribe medications and monitor their effects.

- Provide education and support to clients and families regarding health management and wellness strategies.

- Collaborate with a multidisciplinary team including physicians, therapists, and direct support staff to ensure coordinated care.

- Maintain accurate and timely documentation of patient interactions and care plans.

- Conduct follow-up visits to assess progress and adjust care plans as necessary.
