Job Description

What You'll Do

  • Educate patients on prevention, self-management, and treatment of acute and chronic conditions based on individual health needs, goals, and lifestyle. 
  • Provide proactive, relationship-based care focused on prevention, early intervention, and long-term health outcomes. 
  • Coordinate follow-up care, care management outreach, and continuity of care services to support patient progress and health goals. 
  • Counsel patients on medications, supplements, lifestyle interventions, and potential side effects or interactions. 
  • Order, perform, review, and interpret diagnostic testing including labs, imaging, and point-of-care testing. 
  • Assess patient history, symptoms, diagnostic results, and social determinants of health to develop individualized treatment plans. 
  • Diagnose, treat, and manage acute illnesses, injuries, infections, episodic concerns, and common chronic conditions across in-person, virtual, and employer-based settings. 
  • Collaborate with specialists, behavioral health providers, pharmacies, employer partners, and care teams to support management of complex or high-risk conditions. 
  • Develop evidence-based treatment plans aligned with HealthBar care models and employer-sponsored health initiatives. 
  • Incorporate lifestyle medicine strategies including nutrition, movement, sleep, hydration, breathing, and mindfulness into patient care. 
  • Support employer wellness initiatives, health coaching, engagement programs, and preventative health efforts. 
  • Recommend and coordinate diagnostic, therapeutic, pharmaceutical, behavioral, and specialty interventions with consideration for safety, accessibility, cost, and effectiveness. 
  • Prescribe medications and therapies in accordance with clinical appropriateness, safety standards, and prescribing authority. 
  • Perform primary care and occupational health procedures including wound care, splinting, immunizations, medication administration, drug and alcohol testing, and workplace injury care. 
  • Conduct preventive and occupational health visits including wellness exams, sports physicals, DOT exams, pre-employment screenings, and pre-surgical evaluations. 
  • Coordinate referrals and continuity of care with specialists, imaging providers, therapy services, pharmacies, and ancillary vendors.
  • Collaborate with interdisciplinary clinical, operational, and employer teams to improve patient outcomes, reduce healthcare costs, and support whole-person care.

The (Required) Qualifications

To practice safely and compliantly, we do need:

  • Master’s degree in Nursing from an accredited program
  • Active, unrestricted Illinois RN license
  • Active, unrestricted FNP certification (AANP or ANCC)
  • Active Illinois NP Specialty Certification
  • BLS certification (AHA or Red Cross) within 90 days of hire
  • Must be authorized to work in the U.S. without employer sponsorship
