As part of our growing clinical team, you'll provide advanced wound care services throughout assisted living communities, memory care settings, transitional care programs, and long-term care communities across the Twin Cities.

What You'll Be Doing
• Provide advanced wound care assessments, treatment planning, and ongoing management for complex wounds.
• Conduct routine rounding visits throughout senior living and long-term care communities.
• Collaborate with physicians, nursing teams, therapists, caregivers, and community leadership.
• Help improve healing outcomes while reducing avoidable hospitalizations and complications.
• Educate staff, patients, and families on treatment plans and best practices.
• Support continuity of care across multiple settings and levels of service.
Qualifications
• Active Minnesota Certified Nurse Practitioner (CNP) license.
• Experience in wound care, geriatrics, primary care, post-acute care, skilled nursing, or related settings preferred.
• Strong clinical judgment and confidence managing medically complex patients.
• Valid driver's license and ability to travel throughout the Twin Cities Metro.
